Clinic Closure Notification
It is with warm appreciation and mixed emotions that I write to let you know that RutHormone Clinic PLLC will be closing effective December 2026 ( Finalized official date to be released this week). This decision has been made after thoughtful consideration and is due to personal and family priorities. Caring for you has been a genuine privilege, and I am deeply grateful for the trust you have placed in this practice.
Why You Are Receiving This Notice Now
The primary reason for sharing this update well in advance is simple: to make sure you have plenty of time to establish care with a new provider and to obtain or transfer your medical records before the clinic closes. Starting this process early will help keep your care smooth and uninterrupted. Please do not wait until your medication supply is low or an urgent need arises. Begin connecting with a new qualified healthcare provider as soon as possible so your treatment, prescriptions, and any laboratory monitoring can continue without a gap.
Your Ongoing Care and Prescriptions
If you are currently receiving treatment, taking prescription or compounded medications, receiving hormone therapy, or have laboratory testing that requires follow-up, please arrange care with another qualified provider before your current supply runs out. Your new provider will independently review your history, medications, lab results, and treatment goals to continue your care.
Until the closure date, I will remain your covering provider and the clinic will remain available to support you through this transition, including prescription management, refills, and your questions, up to and through the final closure date. Please note that coverage by this practice ends on the closure date and does not extend beyond it, which is why establishing care with a new provider now is so important. If you have an urgent or emergency medical condition at anytime, please call 911 or go to the nearest emergency department.
A Few Important Housekeeping Items To help this transition go as smoothly as possible, please take a moment to complete the following in Optimantra(link is below this notification): ·
Settle any outstanding balances. Kindly review and pay any outstanding invoices in your Optimantra account at your earliest convenience. ·
Update your mailing address. Please confirm that your current mailing address is up to date in Optimantra so that any written notices regarding the closure —including future details about your records — reach you correctly.
Your Medical Records
Your records will continue to be maintained securely and confidentially in accordance with applicable North Dakota requirements. Your medical records are available for you to download directly from Optimantra at your convenience up until the closure date, so we encourage you to save a copy for yourself and your new provider now. Details regarding the designated medical records custodian — including how to request and transfer your records after closure — will be provided in a future notification. We encourage you to begin planning for your records transfer now so everything is in place before the closure date.
